The participant's information must be EXACTLY the same as the government ID used while traveling (passport or DNI). If you do not have a government ID at this moment, you can fill out the form below now and submit the passport information later. Please make sure that your passport is valid six (6) months past your return date to your country. 

 If you are under 18 years of age, this form must be filled out by a parent/legal guardian.
